I think I need to replace my audio/headphone jack
 

My headphone jack has been getting worse for a while. When my earbuds are plugged in, they have to be in have a certain orientation for the phone to play through them. There's about a 180 degree orientation in the jack that makes the earbuds lose connection and the phone starts playing music through the speakers instead. This is very annoying because it'll get twisted around in my pocket and suddenly there's music coming from my pants. I've tried different eabuds/headphones, so I'm sure it's the jack. How can I find a replacement jack and how complicated is the procedure of replacing it? I'd like to do it myself.
